| You can provide a path from homelessness to hope for our clients by donating a monetary gift to Choices, Inc. Donations in the form of cash, money order, or check may be mailed to: Choices, Inc. 419 S. Shelby St. Louisville, KY 40202 To assist with this process, the Donation Form may be used. Donors who donate at least $10 monthly, or $120 annually, are recognized as Norma's Neighbors and are thanked for their support in the Choices, Inc.
